Observed deeply scored cutting boards not resurfaced or discarded as required. Temperature measuring device for ensuring proper temperature of equipment is not available in the refrigeration unit under the counter area. Exhaust vents are required in both restrooms. Floor tiles behind bar and kitchen area has broken and missing tiles.Holy Rosary School, 125 Stephenson St., Duryea, April 13. A food employee was observed touching ice cream cones &#8212; a ready to eat food &#8212; with bare hands. Observed wet wiping cloths in counter area, not being stored in sanitizer solution. The food facility does not maintain Food Employee Certification records as required. Owner has signed up for upcoming class.Peking Chef, Memorial Highway, Dallas, April 14. Observed boxes and buckets of food stored directly on the floor in walk-in cooler and freezer rather than 6 inches off of the floor as required; this was corrected during this inspection. The handwash sink in the kitchen area was blocked by buckets and not accessible at all times for employee use; this was corrected during this inspection. The food facility does not maintain Food Employee Certification records as required. There was not a current Food Employee Certificate posted. The owner stated that he has scheduled and paid for an employee to take the course on April 20. Owner to send/fax proof of registration and payment for this course to the Region 3 field office or e-mail it to this inspector within seven days.Oregon Inn, 67 Oregon St., Wilkes-Barre, April 14. Wall in the ladies&#8217; bathroom area, has a hole, or is broken and in need of repair. Ceiling tiles missing in the back room area, and need replaced. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed a buildup of lime in the three-compartment sink. Paper towel dispenser empty at the hand wash sink in the men&#8217;s rest room area; paper towels were placed at the sink during this inspection. No sign or poster posted at the hand wash sinks in the kitchen and restroom areas to remind food employees to wash their hands. Signs were posted during this inspection. Observed mouse droppings in bar area personal storage. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate expired Aug. 3, 2014 and is no longer valid. A list of courses was provided to the owner during this inspection.Chuck&#8217;s Main Street Diner, 164 Main St., Luzerne, April 14. Observed a build up of static dust build up on the fan guard in the two-door continental refrigerator in the kitchen.Burger King #1985, 1870 Memorial Highway, Shavertown, April 14. Observed a build up of static dust on the fan guard of the walk-in cooler. Food facility operator failed to post an original, valid Food Employee Certification in a location conspicuous to the consumer.Sophia Coxe Memorial Foundation &amp; Education Center, 2207 Route 940, Drifton, April 15. No violations.Ovalon Bar &amp; Restaurant, 245 N. Wyoming St., Hazleton, April 15. Door gaskets on two-door under counter cooler are duct taped. Surfaces coating on wire shelving in walk in cooler is deteriorated/flaking off. Water filters are not date marked for service intervals.MM Micro Winery, 373 High St., Wilkes-Barre, April 15. Inc. The Mines, 105 N. Main St., Wilkes-Barre, April 15.Di Pisa Italian Restaurant, 101 E. Broad St., Hazleton, April 15. Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Food prep employees observed multi tasking, no hand wash prior to returning to food prep. No hair restraints worn during food prep. Potentially hazardous, ready to eat foods held more than 24 hours are not date marked for 7 day discard. No written procedures available for potentially hazardous foods (pizzas) held at room temperature. No thermometers visible in all in use refrigeration units. Plastic cutting board on bain marie has deep cutting grooves on food contact surfaces. Spray can of WD 40 is stored on food contact surface of meat slicer. Employees observed handling ready to eat foods with bare hands. Assorted knives are stored in homestyle, wooden knife scabbard. Scabbard is not easily cleanable by design. Pot of previously cooked meatballs and sauce on cook top are 126-degree product center; person in charge states product has been on cook top for approximately 3.5 hours and were originally re-heated to 125 degrees, rather than 165 degrees, as required; product was voluntarily disposed due to improper reheating. Approximately 10 pounds of fried chicken wings stored in walk-in cooler and cooked on April 11 are 47 degree product center; product was voluntarily disposed due to improper cooling to 41 degrees within six hours. Assorted knives stored in wooden knife scabbard have old dried food residuals on blades. Cold water faucet handle on food prep hand wash sink is missing. Slicer has build up of old dried food residuals on food contact surfaces; slicer was not used today; slicer disassembled for cleaning/sanitizing during inspection.
