Food dispensing utensil (scoops) in bulk food ingredient containers observed stored in the food and not with handle above the top of the food. Also observed plastic containers with no handle being used as scoops in bulk food containers. Observed clean and dirty utensils stored together in a soiled container. Person in charge placed utensils and container in sink to be cleaned and sanitized. Interior of microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch. Meat slicer, a food contact surface, was observed to have food residue and was not clean to sight and touch. Observed a buildup of a black mold like residue on the fan guard in the Bain Marie. Observed a buildup of grease and dust on air vent on ceiling in rear area of kitchen.Jireh&#8217;s Pizzeria &amp; Restaurant, 416 Main St., White Haven, May 6. No hair restraints worn during food prep. No thermometers visible in all in use refrigeration units. Cooked packages of chicken stored in raw chicken boxes in walk in cooler. Stagnant pooled water on bottom of bain marie unit. He is scheduled to take the course on June 20, 2015. He will fax/e-mail proof that he is registered and paid for the course to the inspector or field office. Main St., Pittston, May 7. Observed knives and metal stem thermometer stored in container of sanitizer. Person in charge removed items from container. Observed hood above cooking equipment, in kitchen area, with an accumulation of dust and grease.Big K-Mart #3268, Route 309, Wilkes-Barre, May 7. The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200 ppm due to improperly functioning dispensing equipment. Facility will use bleach as a sanitizer until equipment can be replaced. Beverage dispensing nozzles on soda and Icee machine, food contact surfaces, were observed to have old syrup residue and was not clean to sight and touch. Person in charge cleaned during this inspection. Observed a buildup of dust and mold like substance on fan in reach in cooler in snack bar area. Observed flour spilled on shelf in the grocery area of the facility. Observed plastic lids for beverages stored alongside handwash sink and subject to splash. Person in charge discarded lids that had splash residue. Severely dented can of ravioli in the back storage area intended for sale in the food facility. Person in charge removed item from stock. Grocery sales area and back storage area of the food facility were dusty, and in need of cleaning. Observed pet food spillage under shelves in sales area facility.
