Born in Dupont, he was a son of the late Edward and Helen Sudosky Slezak. He was owner and operator of John Slezak Auto and Towing Service in Avoca. He was an avid biker and a member of Holy Mother of Sorrows Polish National Catholic Church, Dupont. 
He was preceded in death by several aunts and uncles; and a nephew, David. 
He is survived by his wife of 28 years, Patricia; daughters Loretta, of Pittsburgh; and Stephanie, of Exeter; granddaughter Angeline; brothers Edward Jr., of Wilkes-Barre; Kenneth, of Pittston Twp.; and David, Dupont; sisters Bonnie Wasilewski, Pittston Twp.; Debbie Wall, Jenkins Twp.; and several nieces and nephews. 
Funeral services were Aug. 6 from the Lokuta Funeral Home, Dupont, with a Mass of Christian Burial in Holy Mother of Sorrows PNC Church, Dupont. Burial, parish cemetery.
