The following are recent Luzerne County restaurant inspection reports from the Pennsylvania Department of Agriculture. According the agency: &#8220;any inspection is a &#8216;snapshot&#8217; of the day and time of the inspection. An inspection conducted on any given day may not be representative of the overall, long-term cleanliness of an establishment. Also, at the time of the inspection violations are recorded but are often corrected on site prior to the inspector leaving the establishment.&#8221; The information is taken from the inspection database at www.eatsafepa.com. Postal addresses used here are as listed on the state&#8217;s website, and may not correspond to the municipalities in which facilities are physically located.Black Diamond Tavern, 204 Dana St., Swoyersville, March 14. Regular inspection; in compliance. Violations: Various cracked floor tiles on floor behind bar. Floor is no longer smooth and easily cleanable. The food facility does not have the original certificate for the certified food employee posted in public view.Chili&#8217;s Grill &amp; Bar, 375 Wyoming Valley Mall, Wilkes-Barre, March 14. Follow up inspection; in compliance. Violations: Observed a buildup of ice on cases of food in the walk-in freezer. Mechanical ware-washing equipment had a mineral buildup observed inside. Observed an accumulation of pooling dirty water on the clean side drainboard of the mechanical dishwasher. Mechanical dishwasher had a buildup of black mold-like residue in exhaust hoods. Observed wet wiping cloths in prep area, not being stored in sanitizer solution. Plastic containers and metal pans stored in clean storage areas observed with food and sticky residues on surfaces. Containers and pans were placed at dishwasher to be cleaned and sanitized. Observed a buildup of syrup residue on the surface above dispensing nozzles on beverage machine. Observed a buildup of dust on the fan guard in the True freezer in the kitchen area. Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying.Curry Donuts, 733 Wyoming Ave., Kingston, March 14. Follow up inspection; in compliance. Violation: Webbing buildup on doughnut prep table underside and on underside of the shelves above doughnut prep table.Gerrity&#8217;s Supermarket #490, 552 Union St., Luzerne, March 14. Complaint inspection; in compliance.Patty&#8217;s Sub Division, 743 Main St., Avoca, March 14. Follow up inspection; in compliance.North Slope Brewing Co., 2447 Route 309, Dallas, March 14. Follow up inspection; in compliance. Violation: The receiving door leading outside, located at rear of kitchen, has a gap with visible light between the doors near the floor and does not protect against the entry if insects, rodents and other pests.Wendy&#8217;s #302, 55 S. Wyoming Ave., Edwardsville, March 14. Regular inspection; in compliance. Violations: Cigarette found on the rack in the storage area near the back entrance door during the inspection. Cheddar and American cheese were held at 50 degrees F, in the bane marie next to the grill area, rather than 41 degrees F or below as required. Person in charge stated the foods were put out at 10 a.m. Ice was put under these products and temperatures were brought into compliance at the time of this inspection. Bain marie unit next to grill is not maintaining adequate cool temperature. Person in charge put ice under products to maintain adequate temperatures. Adequate temperatures were attained at time of this inspection. Observed paint chipping on all four hood vents above cooking equipment. Person in charge is putting in a maintenance request to have these surfaces refinished. Observed buildup of black residue on floor behind hot water heater and under ice machine, both non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil.Gumba&#8217;s Pizza, 102 Pittston Ave., Pittston, March 15. Follow up inspection; in compliance.Ole&#8217; Tyme Charley&#8217;s, 31 S. River St., Plains Township, March 15. Regular inspection; in compliance. Violation: Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying.Mohegan Sun Casino - Banquet Kitchen, 1280 Route 315, Wilkes Barre, March 16. Regular inspection; in compliance. Violations: Observed raw eggs stored above ready-to-eat foods in the walk-in cooler. Eggs were moved. Observed case of produce stored directly on the floor in walk-in cooler area, rather than 6 inches off the floor as required. Case was moved from floor. Observed an accumulation of black mold-like grime in vents on both mechanical dishwashers. Observed a buildup of residue on surface above and behind dispensing nozzles on juice and soda machine. Observed food residue on area above shaft on mixer. Observed single-service, single-use articles stored directly on the floor, and not 6 inches above the floor. Items were moved off the floor. Observed a buildup of dust in wall vents behind large dish washer. Observed five small flies in broom storage container by the ware-wash sink. Container was removed from the area.Mare&#8217;s, 1280 Route 315, Wilkes Barre, March 16. Regular inspection; in compliance.Mountain Fresh, Inc., 2203 Route 118, Hunlock Creek, March 16. Regular inspection; in compliance. Violations: No wax declaration statement posted in the produce department of the facility. One partially used container of strawberries stored in the walk-in cooler in the bakery area were moldy. The strawberries were voluntarily discarded during this inspection. Commercially processed, refrigerated, ready-to-eat deli meats and cheeses, time/temperature control for safety food, located in the deli case, and held more than 24 hours, is not being marked with the date it was opened. The meats and cheeses were date marked during this inspection.Mohegan Sun Casino &amp; Racetrack - Northeast Concession, 1280 Route 315, Wilkes-Barre, March 16. Regular inspection; out of compliance. Violations: Food ingredient containers, in the Penny Grove, Breakers, and south areas, are not labeled with the common names of the foods. Containers were labeled. Food dispensing utensil in bulk ingredients observed stored in the food and not with handle above the top of the food. Utensils were removed. Observed cups with no handle being used as a scoop and stored in ice bin in upper main area. Cups were removed. Marinara sauce and hot dogs held at 123 degrees F, in the Starting Gate location, rather than 135 degrees F or above as required. Hot dogs were discarded, marinara was reheated to 165 degrees F. Dairy mixes held at 50 degrees F, in the Poker area, rather than 41 degrees F or below as required. Items were discarded. Temperature measuring device for ensuring proper temperature of equipment is not available in multiple refrigeration units in the Poker, Center and Starting Gate area. Refrigeration unit in the Poker area observed with an ambient temperature of 50 degrees F and not 41 degrees F or less as required. No TCS items will be stored in unit until it can be serviced and maintains appropriate temperature. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing glass dishwashers in multiple bars was 0-25 ppm, and not 50-100 ppm as required. Facility will wash glasses in high temperature dishwashers until glass washers can be serviced. Observed temperature gauges on multiple glass washers not working. Interior of microwave in the South Bar was observed to have food residue and was not clean to sight and touch. Microwave was cleaned. The Person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food employees observed in Starting Gate prep area, not wearing proper hair restraints, such as nets or hats. Observed 2 gallons of chocolate milk and 1 gallon milk with expiration dates of March 15 and March 13. Items were discarded. Observed deeply scored cutting boards not resurfaced or discarded as required. Observed a buildup of food residue on areas around dispensing nozzles on multiple Cappuccino machines. Observed a buildup of food residue on bottom shelf in cabinet at Breakers. Shelf was cleaned. Observed a buildup of grime and rust in multiple sinks and locations. Observed a buildup of grime on the exterior by controls of multiple glass washers. Observed a buildup of dust on fan guards in refrigeration units in Penny Grove, South, Center, and North locations. Observed a buildup of food residue on surface above and behind dispensing nozzles on multiple beverage machines. Observed single-service, single-use articles in multiple storage/bar areas stored directly on the floor, and not 6 inches above the floor. Items were moved off the floor. No sign or poster posted at the hand-wash sink in the multiple locations to remind food employees to wash their hands. Soap was not available at the hand-wash sink in the multiple locations. The hand-wash sink in the Breakers, South, and Starting Gate blocked by equipment and not accessible at all times for employee use. Old food residue, and/or equipment observed in multiple hand-wash sinks at various locations, indicating uses other than hand washing.Rita&#8217;s Italian Water Ice, 355 Market St., Kingston, March 16. Regular inspection; in compliance. Violation: Soap was not available at the hand-wash sink in the men&#8217;s employee restroom. Soap was placed during this inspection.Bill&#8217;s Cafe, 32 Old Lake Rd., Harveys Lake, March 16. Regular inspection; in compliance. Violation: Food facility is using an approved non-public water system, regulated by the state Department of Environmental Protection, but does not have current laboratory testing results for water potability available in the facility. The Person in charge stated that All Mechanical comes in monthly to draw water samples and take to Kirby Health Center and stated that they were here last and took samples on Friday. The facility will email to this inspector or fax or mail to The Department&#8217;s Region 3 office by March 30, 2016.Dallas Senior Center, 1158 Twin Stacks Dr., Dallas, March 16. Regular inspection; in compliance.Dollar General #9423, 166 N. Memorial Hwy. Shopping Center, Shavertown, March 15. Follow up inspection; in compliance.Dowes&#8217;s Suds &amp; Grub, 914 Exeter Ave., Exeter, March 16. Follow up inspection; in compliance.Dukus&#8217; Cafe, 254 Pringle St., Kingston, March 15. Regular inspection; in compliance. Violations: Thermometers for ensuring proper temperatures of food are not available or readily accessible in all refrigeration/freezer units. All temperature in all units were 41 degrees F or below. The door located at the landing of the basement steps and leading to the outside, has a gap with visible light at its base and does not protect against the entry of insects, rodents, and other animals. The gap is approximately 2 inches by the width of the door. Observed a buildup of dried food residual on the frame (a non-food contact surface) of the deli-slicer in the food prep area. Areas of floor tiles missing and damaged in kitchen. These locations no longer smooth and easily cleanable. Wall in the dry storage room, behind and to left of hand-wash sink, has an area that the paneling is missing and lath exposed. The food facility does not have the original certificate for the certified food employee posted in public view. Person in charge contacted the owner&#8217;s sister via telephone who stated that both she and the owner have current Food Employee Certificates but could not locate them. The facility will e-mail copies of certificates to this inspector or fax to the Region 3 Office within five days.<style>
