Observed a buildup of black grime on the fan guard inside the white reach-in beer cooler in the self-service area. The door to enter/exit the food facility is being propped open. There is no screen door. The door was closed during this inspection. The door shelves inside of the reach-in upright freezer in the prep area, a food contact surface, was observed to have food residue and was not clean to sight and touch. Violations: Bulk food ingredient storage containers, in the deli area, not labeled with the common name of the food. Containers were labeled. Observed a buildup of ice in the walk-in grocery cooler. Observed a buildup of water from mister in trays holding vegetables. Trays were removed. Soup was held at 121 degrees F, in the self-serve area, rather than 135 degrees F or above as required. Soup was reheated to 165 degrees F and placed in hot hold. Temperature was adjusted on the hot hold unit. Hot dogs were held at 48 degrees F, in the refrigeration case near deli area, rather than 41 degrees F or below as required. Foods were discarded and unit shelving was adjusted. Prepackaged foods are not labeled properly with the full ingredient statement indicating allergen ingredients. Observed a buildup of rust on the shelves in the walk-in cooler in the deli area. Observed a deteriorating gasket on proofer in bakery area. Observed deeply scored and stained cutting boards not resurfaced or discarded as required. Produce scale, a food contact surface, was observed to have food residue and was not clean to sight and touch. Scale was cleaned. Observed a buildup of slimy residue and debris on produce shelves in sales area. Shelves were cleaned. Observed a buildup of dust on vents on dairy and deli cases. Observed an accumulation of debris under shelves in the produce and deli areas. Observed an accumulation of debris on the floor in multiple areas (walk-in coolers, meat room, deli area, back storage areas) of the facility. Observed a buildup of residue on the non-food contact surfaces of the mixer in the bakery area. Observed single-service, single-use articles stored in multiple area directly on the floor, and not 6 inches above the floor. Items were moved. Multiple sinks have a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Observed hand-wash sink in the produce area not working properly. Sink was repaired. Paper towel dispenser empty at the hand-wash sink in the bakery and meat room area. Paper towels were provided.<style>
