DEA issues warning regarding risks of carfentanil

<p>Two member of the Wilkes University faculty have been awarded a $150,000, three-year grant to help improve patient transition from hospital to home.</p><p>Judith Kristeller and Dana Manning of the Nesbitt School of Pharmacy received the grant from the Cardinal Health Foundation. The money will be used to help with transition of care and medication use for patients discharged from Commonwealth Health hospitals, a for-profit company that owns and operates several facilities in the area, including Wilkes-Barre General.</p><p>The two won a separate, $41,000 grant from the Moses Taylor Foundation for the same project.</p><p>Kristeller, a professor of pharmacy practices, developed a model connecting hospital and community pharmacists as well as physicians to ensure patients are educated about their medications and receive follow-up care after leaving the hospital.</p><p>The model has been piloted at Moses Taylor hospital in Scranton, and the grants will help expand it to Regional Hospital of Scranton.</p><style>
