MOUNTAIN TOP - The Crestwood School District is holding kindergarten registration for the 2017/2018 school year at Rice Elementary and Fairview Elementary schools. Registration times for both schools are 9 a.m. to noon and 1 to 3 p.m. Enrollment should take approximately an hour.Registration for Fairview Elementary is April 18 and 19, 117 Spruce St. Rice Elementary is April 20 and 21, 3700 Church Road.Children should be 5- years old by Sept. 1, 2017 and are required to accompany parent or guardian for academic, speech and vision screenings. Required documents needed at time of registration include the child&#8217;s birth certificate, current immunization records plus three (3) proofs of residency showing name and current address. Examples would be a deed to home, agreement of sale, lease containing your name and verified by building owner, real estate tax receipt, valid PA Department of Transportation license or identification with current address within Crestwood School District, W-2 statement/IRS statement or tax return, homeowner&#8217;s/tenant&#8217;s insurance statement, copy of state/federal program enrollment, voter registration card noting township or borough, current utility bill (gas electric, water, sewer or verification of service start date), check stub from employer showing wages, public assistance, or social security, builders contract.In accordance with district policy, in cases where the legal guardian is not the lessee or homeowner, but is residing with someone who is, above documents are needed along with a notarized affidavit.
